目的：研究对急性缺血性脑卒中患者应用重组组织性纤溶酶原激活剂（rt-PA）静脉溶栓治疗的临床价值及对治疗有效率、凝血指标水平的影响。方法：抽选于2020年10月-2022年3月期间的急性缺血性脑卒中患者80例，随机划分组别，即对照组、研究组，各40例。对照组接受常规治疗，研究组接受rt-PA静脉溶栓治疗。评价及对比两组的治疗有效率、凝血指标水平、神经功能、脑血流动力学。结果：研究组的治疗有效率高于对照组（P＜0.05）；与对照组相较，研究组的各项凝血指标水平均明显改善（P＜0.05）；研究组的神经功能受损量表（NIHSS）评分低于对照组（P＜0.05）；研究组的动脉最小血流速度（Vmin）、动脉最小血流量（Qmin）均高于对照组，脑血管外周阻力（R）低于对照组（P＜0.05）。结论：对急性缺血性脑卒中患者应用rt-PA静脉溶栓治疗，可明显提高疗效，改善凝血功能，减轻神经功能受损程度，促使脑血流恢复。
