颅内巨大动脉瘤（直径超过2.5cm）是神经外科领域最具挑战性的病变之一，其治疗策略远复杂于普通动脉瘤。本文系统综述了颅内巨大动脉瘤手术治疗的发展历程、现有技术及未来方向，涵盖了显微外科手术、血管内治疗及复合手术等多种治疗策略。通过对现有文献的分析表明，现代颅内巨大动脉瘤治疗已从传统的瘤颈夹闭逐步发展为包括载瘤动脉重建、血流导向及旁路移植等多种技术的个体化综合治疗模式。尽管手术治疗技术显著进步，但颅内巨大动脉瘤的并发症风险仍较高，需要严格的术前评估和术后管理。未来，随着新型材料技术、血流动力学评估手段及微创技术的不断发展，颅内巨大动脉瘤的治疗效果有望进一步提升。
